BC Patient Medical Home Expectations: Workshop

Designed to support the Patient Medical Home Expectations Task Group, this session gave participants an opportunity to provide input on the patient medical home attributes of contact (timely access) and comprehensive care (care of patients across the life-cycle, care across clinical settings and geographic areas, and care across the full spectrum of services). Discussion notes from the session will be used to inform expectations for the fully realized model.

There were three principle themes across all topics: 
 

  • Patients’ expectations need to be managed.
     

  • Comprehensive care is not just the responsibility of the patient medical home (it needs to be delivered across a network of physician, community, and HA services).
     

  • In order for comprehensive care to be delivered, seamless networks with excellent communication, shared information, and clear referral pathways need to be in place.
